HOW TO USE?
How do you use the NAD+ Longevity Pen?
The pen is designed for subcutaneous self-administration according to the dosing protocol included with your order. Read the complete instructions before first use.
- Choose a clean injection site each time, rotating locations as guided in the protocol
- Use a new sterile needle for every single administration — never reuse needles
- Administer once daily, or according to the schedule provided — do not exceed the recommended dose
- Store cartridges refrigerated, protected from light and heat, including during travel
- Do not share the pen or needles with anyone
- Consult the Sarasin team or your physician before first use if you take medication or have existing health conditions

ABOUT THE FORMULA
What NAD+ is, why it declines, and why delivery route matters.
NAD+ is a coenzyme present in every cell and involved in hundreds of metabolic reactions. It carries electrons in the mitochondrial processes that convert nutrients into ATP, the form of energy cells run on. It is also the primary substrate for two critical enzyme families. Sirtuins (SIRT1 through SIRT7) are NAD+-dependent enzymes that regulate gene expression, mitochondrial biogenesis, metabolic adaptation and stress response pathways that determine cellular resilience over time. PARP enzymes use NAD+ to detect and repair DNA strand breaks. When NAD+ falls, both systems lose their substrate and their effectiveness falls with them.
The subcutaneous delivery route is designed to introduce NAD+ directly into tissue beneath the skin, where it enters the interstitial fluid and is absorbed into systemic circulation. This bypasses the gastrointestinal system, which breaks down orally administered NAD+ into metabolite fragments before absorption. The same pharmacological principle underpins insulin and many medical biologics: subcutaneous delivery is used specifically for compounds that are degraded by the gut. The clinical extent of the bioavailability advantage of subcutaneous NAD+ over oral supplementation in humans has not yet been established in direct comparative trials, and we state this clearly below.

What is established in human research
The age-associated decline of NAD+ in human tissue is well established. Massudi and colleagues (2012) measured NAD+ across age groups in human tissue and found a significant decrease alongside rising oxidative stress markers. Covarrubias and colleagues (2021) reviewed human and animal data in Nature Reviews Molecular Cell Biology and identified NAD+ depletion as a driver of multiple hallmarks of ageing. These are findings grounded in human biology.
Human clinical trials with NAD+ precursors NMN and NR have demonstrated that supplementation raises NAD+ metabolite concentrations in blood and skeletal muscle. Elhassan et al. (2019) showed NR raised NAD+ in blood and muscle in healthy older adults with anti-inflammatory transcriptomic changes. Yoshino et al. (2021) demonstrated that NMN improved skeletal muscle insulin sensitivity in postmenopausal women with prediabetes. Martens et al. (2018) confirmed NR was well tolerated and elevated NAD+ in healthy middle-aged and older adults. Intravenous NAD+ has also been studied directly: Grant et al. Distinguishing levels of evidence: human, animal and laboratory
In vitro studies have established with high confidence how NAD+ supports mitochondrial function, sirtuin activation and DNA repair. They tell us the biology but cannot confirm clinical benefit in living people. Animal studies, primarily in mice, show that raising NAD+ can extend healthy lifespan and improve metabolic function — compelling findings that have driven the human research agenda, but not directly transferable to human outcomes. Human clinical trials are the highest level of evidence. The human database is growing but remains smaller and more heterogeneous than the animal literature. The most consistent human finding is that supplementation raises NAD+ metabolite levels. Which specific clinical benefits follow in healthy adults, at which dose, and over what timescale, are still being established. All claims on this page are grounded in what human evidence supports. Subcutaneous versus intravenous versus oral: the honest picture
No published clinical trial has directly compared the absorption, bioavailability and clinical outcomes of subcutaneous NAD+ administration against IV NAD+ or oral precursors in the same human subjects. This direct comparative evidence does not yet exist, and we state this without qualification.
What the available evidence shows is this: IV NAD+ raises plasma NAD+ metabolites significantly and rapidly in humans (Grant et al., 2019). Oral NAD+ and precursors are processed by the gut before systemic absorption, with much of the administered compound converted into metabolite fragments en route. Subcutaneous delivery bypasses this gastrointestinal conversion — the pharmacological rationale is sound and the route is established medical practice for many injectables. Whether this translates into meaningfully superior outcomes versus IV or well-dosed oral precursors in humans is a question that published comparative trials have not yet answered. NAD+ directly versus precursors: NMN and NR
NAD+ precursors such as NMN and NR are not NAD+ itself. They are molecules the body converts into NAD+ through enzymatic steps inside the cell. Most published human clinical trial evidence on raising NAD+ through supplementation involves these precursors, not direct NAD+ administration. The Longevity Pen delivers NAD+ as the complete molecule. When taken orally, NAD+ is largely broken down in the gut before absorption; subcutaneous delivery introduces the complete molecule directly into tissue, bypassing this conversion. Whether the cell uses directly delivered NAD+ differently from precursor-derived NAD+ is an active area of research. Published head-to-head human data comparing direct NAD+ delivery with precursor supplementation remains limited, and this is stated clearly.
